Swing Bed Program
Supportive, Skilled Recovery Before Going Home
The Swing Bed Program provides short-term skilled nursing and rehabilitation services for patients who no longer need acute hospital care but are not yet ready to return home.
This program offers a smooth transition from hospital to home in a familiar, supportive setting.
Swing Bed care may include:
- Skilled nursing care
- Physical, occupational, or speech therapy
- Medication management
- Discharge planning and care coordination
The goal is to help patients regain strength, independence, and confidence before returning home.
