Swing Bed Program

Supportive, Skilled Recovery Before Going Home

The Swing Bed Program provides short-term skilled nursing and rehabilitation services for patients who no longer need acute hospital care but are not yet ready to return home.

This program offers a smooth transition from hospital to home in a familiar, supportive setting.

Swing Bed care may include:

  • Skilled nursing care
  • Physical, occupational, or speech therapy
  • Medication management
  • Discharge planning and care coordination

The goal is to help patients regain strength, independence, and confidence before returning home.

  • Post-surgical recovery & complex wound management

  • IV therapy

Rehabilitation Services

  • Physical Therapy: Regain balance, strength, and mobility

  • Occupational Therapy: Return to everyday tasks with confidence

  • Speech-Language Pathology (SLP): Support for communication, memory, and swallowing
