At MCH, you have a right to access your health information when you need it. Our Medical Records Department is here to help you request copies of your records or share them with other providers in a secure and organized way.

How to Request Your Medical Records

To obtain copies of your health records:

• Fill out the Consent for Release of Information form (available below).
• Return the completed form by email or mail:

Please note we do not accept electronic signatures on record requests.

There may be a copy fee for medical records (excluding immunization records). Records can be sent directly to your new doctor or medical facility free of charge if requested.

You can access your immunization records directly through the Nebraska DHHS immunization portal.
